Republic Bank offers Associates the Delta Premier PPO Plus plan through Delta Dental of Kentucky. The
dental plan is a unique blend of
Delta Dental's Premier and Preferred provider networks. If you use a dentist from the Preferred provider
network, your out-of-pocket costs
are lower because these network providers offer a greater discount for their services. All Full-Time and
eligible Part-Time Associates are eligible
to participate effective on their first day of employment.

plan through Humana Vision Care.
Flexible Spending Accounts provide tax advantages that can help pay health care and dependent care expenses on a pre-tax basis. Limited Purpose FSA plans are designed to work in conjunction with Health Savings Accounts for Associates enrolled in a High Deductible Health Plan (HDHP). By anticipating your family's health care and dependent care costs for the next year, you can lower your taxable income. 2023 FSA Quick Start Guide
The Republic Bank HSA is a free - no fee tax-favored account that allows you to set aside funds to save
and pay for qualified medical expenses incurred by you, your spouse, and any of your qualified dependents. The HSA takes the form of a
tax-exempt trust or custodial account.
All Full-Time and eligible Part-Time Associates are eligible to participate effective on their first day

Republic Bank Health Savings Account
As a Full-Time or eligible Part-Time Associate, effective on your first day of employment, Republic Bank provides
(at no cost to you) basic group term life and AD&D benefits, as follows:
- Full Time - 2x annual base pay (maximum benefit amount is $300,000)
- VP - $250,000
- SRVP - $450,000
- Part-Time - $15,000
In addition, all covered Associates are provided accidental death and dismemberment (AD&D) coverage
equal to the same amount of their basic
life benefit. Example, if you should die as the result of a covered accident, your beneficiary would
receive an additional benefit equal
to your basic group life benefit amount.
You can also purchase additional life insurance for yourself, your spouse, and your eligible children at
affordable group rates. All Full-Time and eligible Part-Time Associates are eligible to participate
effective on their first day of employment.

Evidence of Insurability
Republic Bank provides short-term and long-term disability income benefits and pays the full cost of this
coverage. As a Full-Time Associate, you become eligible for disability benefits following 180 days of
employment.
Cancer, Critical Illness and Accident policies are available at discounted rates through pre-tax payroll deduction.
AFLAC pays cash benefits directly to you to help with expenses associated with a covered illness or injury.
Coverage is available for yourself, your spouse, and children under age 26.
To help you prepare for the future, Republic Bank sponsors a 401(k) Retirement Plan that lets you save
and invest pre-tax and after-tax dollars from your pay. You are eligible to participate in the plan
immediately upon employment; at which time you will also be eligible for the Company Match.
The Company will match the first 1% of pay you contribute at 100%. The next 2-5% of pay you contribute
will be matched at 75%. You are always fully vested in your own contributions, as well as any investment
earnings on your contributions. Republic Bank's matching contributions are vested after two years of
employment.
New Associates are eligible to participate in the Republic Bank Employee Stock Purchase Plan (ESPP)
after 6 months of employment. By participating in the ESPP, you have the opportunity to own Corporation
stock by accumulating funds to do so with convenient after-tax pay-roll deductions. In this way, you
share in the Corporation's financial performance. The ESPP allows you to set aside between $10 to $490 from
each paycheck, on an after-tax basis over a 3-month offering period. The stock is purchased at a 15%
discount on the stock price at the end of the 3-month offering period using your contributions.

Paid Time Off (PTO): Republic Bank has designed our Paid Time Off (PTO) benefits
to provide you with as much flexibility as possible when you need paid time off. Use PTO to enjoy a
vacation, take some personal time for any reason, or an occasional sick day.

Parental Leave: Republic Bank provides up to two weeks of 100% paid parental leave for Associates who are expanding their families. This includes birth, adoptive and foster parents, regardless of sexual orientation or gender.

holidays listed below, and any other holidays as designated by the Board of Directors. If the
holiday falls on a day the Associate is normally scheduled to work, the holidays listed below

Bereavement Leave: If an Associate wishes to take time off due to the death of
an immediate family member, paid bereavement leave will be provided to eligible Associates.

As an Associate of Republic Bank, you are eligible immediately upon hire. The plan provides benefits to
